Facilities Request Form

Use this form to request church facilities for your event.

NOTE: If your event is occurring less than a week from the time you submit this form, please call 256-539-0622 ext. 241 in addition to filling out the form. This way we'll be sure to prepare for your event in time.

Note any extra equipment needs you might have (e.g. VCR player, wireless mic, etc.), as well as any special instructions regarding your equipment.

If you are using any A/V equipment in the sanctuary, you will need to pay $15.00 per hour for a Sound Technician. The total number of hours you request should cover the duration of your event, plus an extra hour for set up/tear down. Payment can be submitted to the Worship Ministry.

If you need a sound tech, this request should be submitted at least 20 days prior to the event.

Is there any additional equipment you need?

In order for childcare to be provided, your request must be turned in no later than two weeks prior to the event.
